Acute or chronic pain:

List etiology if known (e.g., trauma, cancer, post-procedural, post-thoracotomy, chronic with psychosocial dysfunction)
Document site and laterality

Central pain syndrome: list etiology or underlying cause, if known.

Surgical complications

Include the cause-and-effect relationship between the care provided and the complication.
Indicate that the condition is a complication.
Document the time frame when the complication occurred (e.g., intra-operatively, post-operatively).

Tobacco

Improving physician documentation specificity

Differentiate between tobacco use/abuse or dependence (i.e., addiction to tobacco products caused by the drug nicotine).
Document type of tobacco product (e.g., cigarettes, chewing tobacco, or cigars).
Differentiate between patients who no longer smoke and those that do. Instead of history of smoking, use former smoker.
